Common Gaps in Expats Health Insurance (And How to Avoid Them)

You probably think your health insurance covers you abroad. Most US expats do.

Then something happens. A sudden illness in Bangkok. A skiing accident in the Alps. A heart attack in Buenos Aires.

That’s when people discover the truth. Their coverage has holes big enough to bankrupt them.

Let’s talk about what gets missed. Because knowing these gaps in expats health insurance now could save you from a financial nightmare later.

    The Pre-Existing Condition Trap

Here’s what catches people off guard. Many expats health insurance plans exclude pre-existing conditions for the first 12 to 24 months. You might think your diabetes or high blood pressure is covered. It’s not. At least not right away.

This creates a dangerous window. What if you need medication adjustments? What if complications arise? You’re paying premiums but getting limited protection when you need it most.

Some plans never cover pre-existing conditions at all. Others require medical underwriting that can deny coverage entirely. The fix? Read the fine print before you move. Ask specific questions about your conditions. Get answers in writing. Don’t assume anything.

    Emergency Evacuation Gets Overlooked

Medical evacuation sounds dramatic. Like something that only happens in movies.

But it’s more common than you think. Remote locations with limited medical facilities exist everywhere. Even major cities sometimes lack specialized care for serious conditions.

Air ambulance costs run between $50,000 and $200,000. Ground transport across borders adds up fast, too.

Many basic international medical plans skip this coverage completely. They’ll pay for treatment at the nearest hospital. But getting you to a facility that can actually help? That’s on you.

Check if your plan includes emergency evacuation. Make sure it covers transport to your home country if needed. This single benefit can prevent financial catastrophe.

    Mental Health Coverage Falls Short

Depression. Anxiety. Burnout. These don’t stop just because you moved overseas.

Yet mental health coverage in international plans often gets minimal attention. Some policies cap therapy sessions at 10 or 20 per year. Others exclude mental health entirely unless you’re hospitalized.

Living abroad brings unique stressors. Culture shock is real. Isolation happens. Work pressure increases when you’re far from familiar support systems.

You deserve proper mental health support. Look for plans with substantial outpatient mental health benefits. At least 30 sessions per year. Coverage for both psychiatrists and therapists.

Don’t settle for plans that treat mental health as an afterthought.

    Maternity Benefits Come with Surprises

Planning to start a family abroad? Or already pregnant?

Maternity coverage varies wildly. Some plans require a waiting period of 12 to 18 months before they cover pregnancy. Others exclude it completely.

Complications make things worse. High-risk pregnancies. Emergency C-sections. Premature births require NICU care. These scenarios pile up costs fast.

A standard delivery abroad might cost $5,000 to $15,000. Complications can push that to $50,000 or more.

Check maternity benefits carefully. Look at waiting periods. Understand what’s covered and what’s not. Consider adding maternity riders if your base plan falls short.

Dental and Vision: Get the Bare Minimum

Most international medical plans treat dental and vision as extras. Basic plans might cover emergency dental only. Routine cleanings? Fillings? Eye exams? Often excluded.

This seems minor until you’re dealing with a cracked tooth in Singapore. Or need new glasses in Germany. Out-of-pocket costs add up quickly.

Perhaps dental and vision feel less urgent than major medical coverage. But going years without proper care creates bigger problems down the road.

Look for comprehensive plans that include routine dental and vision. Or budget separately for these expenses if your plan doesn’t cover them.

    Geographic Restrictions Create Problems

Your plan might cover you in Europe but not Asia. Or vice versa.

Geographic limitations trip up expats who travel frequently. Digital nomads face this constantly. Business travelers crossing regions run into coverage gaps.

Some plans exclude the US entirely. Others limit US coverage to emergency care only. This becomes a problem when you visit family or need specialized treatment back home.

Read the geographic coverage section. Know exactly where you’re protected. If you travel across multiple regions, choose a truly global plan.

    Chronic Disease Management Gets Expensive

Living with a chronic condition abroad requires ongoing care. Regular check-ups. Prescription refills. Lab work. Specialist visits.

Many plans cover acute episodes but provide weak support for chronic disease management. You might get coverage for a heart attack. But the cardiology follow-ups afterward? Limited.

This gap forces people to choose between proper care and financial stability. Neither option feels good.

Find plans with strong chronic disease support. Look for unlimited specialist visits. Coverage for routine monitoring. No arbitrary visit caps.

What You Can Do Right Now

Start by reviewing your current coverage. Line by line. Don’t guess what’s included.

    Ask these questions:

What’s the waiting period for pre-existing conditions?

Does the plan include emergency evacuation?

Is routine dental and vision included?

Which countries and regions are covered?

What support exists for chronic disease management?

Get answers before you need the coverage. Because when an emergency hits, it’s too late to fix gaps.
